The following is a basic outline of the consultative process:
- Office contact. (Call 415-677-5909, or fax 415-391-0287)
- Fill out the questionnaire and return it with
any appropriate medical information.
- Payment of an initial fee of $100 for a record review and subsequent phone consultation.
- After the above are completed, the initial phone consultation will be scheduled.
- Discussion of the problem and the strategies to solve the problem.
- Future appointments if necessary.
A sliding fee schedule ($50 to $250) will be utilized after the first phone consultation of $100. Most insurance carriers will not cover this service. On occasion, others will be assisting me, which may result in additional fees.
